Players of The Sims 4 have created numerous mods and add-ons to enhance their gaming experience, ranging from farming-themed additions to those related to transgender topics. Although the game already allows Sims to experience attraction to any gender, there is no official content that provides a genuine transgender playthrough. Some of these mods may no longer function properly. Hence, we have compiled a list of suggestions for players who want to create transgender Sims without solely relying on Sims 4 transgender mods.
The Sims 4 mod called Zero’s Mental Health Mod is a popular and regularly updated option that lets players create and customize mental health issues for their Sims, including Gender Dysphoria, which can be resolved through Hormone Replacement Therapy. The mod offers several choices for players to help their Sims deal with the disorder, such as attending counseling, joining support groups, or relying on harmful coping mechanisms like substance abuse. It’s important to note that this mod is not intended to suggest that being transgender is a mental illness or to stigmatize mental health issues.
Zero has also developed a Sims 4 transgender mod called the Hormone Replacement Therapy (HRT) mod that enables players to simulate the process of transitioning using their Sims. Transgender Sims can buy three new consumable items through the computer, allowing players to replicate the effects of undergoing HRT.
Players must obtain a doctor’s prescription in The Sims 4 game to access hormone treatment. They can do this by researching hormone replacement therapy (HRT) online through a Sim’s phone or computer and then scheduling an appointment with a doctor. The prescription costs 375 simoleons. It’s important to mention that the HRT mod is meant to be used in conjunction with the Wicked Whims mod, and if a sim begins taking testosterone, they will cease having periods in the Wicked Whims mod.
Sometimes in Sims 4, players combine multiple mods or official expansions to achieve the desired features. It is also the case with Meep62’s Gender Transition mod, which works well with Zero’s HRT mod.
Meep62’s Gender Transition Mod for The Sims 4 offers three versions that allow players to add realistic top and bottom surgery options. The first version includes a “Go to Voice Training” option to change your Sim’s voice from masculine to feminine and vice versa, as well as an instant “Take Hormones” interaction, but the gradual effects provided by Zero’s HRT mod are missing. The second version automatically changes your Sim’s voice when they take hormones, while the third version removes the voice training altogether. If you already have mods that include some of these features, you may prefer the simplified version of Meep62’s Gender Transition Mod.
PimpMySims4’s LGBT Mod is another comprehensive mod for The Sims 4 that allows players to create an identity for their Sims in Create A Sim by adding traits such as Aromantic, Asexual, Bisexual, Demisexual, Gay, Gender Fluid, Lesbian, Non-Binary, Pansexual, and Transgender. Additionally, Sims can be given characteristics like Ally, LGBT Activist, Drag Queen/King, or Questioning, which adds more depth to their self-discovery stories. The mod is available in several languages, including English, Polish, German, and Russian. For those seeking a more authentic experience, the mod also offers a homophobic/transphobic feature that causes other Sims to feel uncomfortable around your Sim.
Players who want to enhance the experience of their transgender Sims in The Sims 4 without installing mods can use free custom content available online. Custom content like Cloudy Simmers’ Female Trans Scars and Male Trans Scars can be added to give the impression of gender confirmation surgeries. Schmapple’s Binders can also be used to give Sims the appearance of wearing a binder, although they are not functional. Create A Sim also offers options for transgender Sims, such as switching between male and female body types, specifying pregnancy options, and selecting peeing positions.
